Your Health History

Your Symptoms

Write down:

  • Any new feelings in your body

  • Where they feel strongest

  • What makes it better or worse

You and Your Close Family's Health History

Write down:

  • Health problems you have now

  • Health problems you had in the past

  • Genetically related family members with high blood pressure, heart problems, or diabetes

  • Recent changes to your health

Tell your doctor about:
